MISSION
The sports medicine program functions to serve the CSUEB intercollegiate student-athletes. The mission of the sports medicine program is to enable the student-athletes of CSUEB to reach their athletic potential by preventing, managing, treating and rehabilitating athletic injuries using evidence-based information and consulting with adjunct health care professionals.  The sports medicine staff is dedicated to helping the CSUEB student-athletes return to competition as soon as and in the safest manner possible. Staff will conduct themselves in a professional manner and follow the standards set forth by CSUEB, the National Athletic Trainer’s Association (NATA), the National Collegiate Athletic Association (NCAA), and the California Collegiate Athletic Association (CCAA).

New Student-Athlete Information
  • In order to participate in athletics all documents must be completed before you can participate. You will receive a invitation via Healthy Roster in June of your incoming year where you can complete all the documents below.
  • Pre-participation physicals completed by an MD or DO, this can be done with your family doctor, from a local clinic or on campus with our team physicians. Physical must be completed after April 1st of your incoming year.

Secondary Insurance
CSUEB holds a secondary excess policy that covers athletes participating as a member of the team in a regularly scheduled practice, game, scheduled workout session, supervised and sponsored sports activity or covered sports travel (CARA). 
 
SA are eligible for secondary insurance access if they are active on the CSUEB roster maintained by the compliance director and have completed all sports medicine clearance requirements. 
  • If a SA injury requires the use of CSUEB secondary policy, the SA will be notified by the evaluating AT to email PDF or upload copies of bills, explanation of benefits, or other billing paperwork like ledgers to the Sports Medicine Staff within 90 days from date of service. Failure to do so will result in the student-athlete incurring the cost of the medical expenses.
  • Bills that have gone into collections because they were not submitted in a timely manner are not the responsibility of CSUEB.
  • If a student-athlete desires to see an outside physician regarding an injury or illness, he or she must first receive authorization from the team physician or Sports Medicine Staff.  The athlete will incur the cost of the services of that physician if prior authorization was not received.
The athletic department does not automatically assume financial responsibility for any medical bills.  If proper referral and insurance procedures are followed, the department’s excess coverage plan will be made available to file claims against.  The injury must be related to intercollegiate participation at CSUEB.  Any pre-existing condition is not covered under the plan.  Failure to adhere to the outlined referral and primary insurance policies may result in a loss of the secondary athletic insurance eligibility resulting in the SA incurring the costs of the medical expenses.
